TO THE COTON OWNER:

• These health tests should require about 30 minutes or less of your veterinarian's time, so please tell the staff that when you make an appointment (most office visits are for about 15 minutes).
• Bring two rulers and a flexible tape with you in addition to all three forms (this form, the Blood Chemistry Form and the General Office Exam form).
• Please note that all three forms must be current. Therefore, check the dates on the form you want to use and, if necessary, download new forms if it has been thirty days or more since your downloaded these forms.
• When you have all three forms and your veterinarian has completed Form 2 and Form 3, please make two copies of everything (forms and all lab reports and veterinarian notes to the exam):
  • Keep one complete copy for your records;
  • Mail one complete copy to the breeder of the dog, and;
  • mail the original, completed forms, lab reports and notes to:

TO THE VETERINARIAN:

Thank you for your help with the CTCA's Health Test. These tests are designed to help us determine whether or not this dog should be granted "BREEDABLE" status. Such a classification is NOT written in stone, of course, and is subject to change should we learn something about the health of this dog in the future that would require a reclassification.

You are an integral part of our club's effort to insure that the Coton de Tulear Club of America's population of dogs is healthy and happy. We sincerely appreciate your input on any matter impacting this breed. We have designed these forms to be simple and direct. We realize that no one can diagnose every possibility; we only ask you to put down as best you can what you observe. If you feel that the dog should be seen by a Board Certified specialist in any area of these tests, please note that on the form and inform your client. The Coton de Tulear dog is a rare breed that we have studied and parented for more than a quarter century. We permit no mating that would produce puppies more inbred than 10% (current Code of Ethics, determination via Wright's Coefficient of Inbreeding). We sincerely appreciate your contributions to the welfare of this dog and the health of our small but well considered gene pool.
